Hometown Health and Sierra Health-Care Options, Nevada’s leading Managed Care
Organizations, have partnered together to provide a statewide comprehensive preferred provider
network to the participants of the Public Employees’ Benefits Program (PEBP) Self-funded
PPO Plan.
To gain the most from your health benefit package, please read carefully the materials provided
by PEBP. The following tips will help you maximize your benefits under this Plan:

- Know the requirements of your Plan regarding referrals, pre-authorizations, and the
limitations of your coverage.
- Access healthcare services from contracted Plan Providers in this directory. You may
also search for Plan Providers online at www.pebp.state.nv.us.
- Contact your Medical Management Company, APS Healthcare, at (888) 323-1461 for
services that require pre-authorization.
- In Southern Nevada, to obtain additional information and/or referrals regarding
behavorial health/substance abuse providers, please contact Behavioral Healthcare
Options (BHO) at (702) 364-1484 or (800) 873-2246.
- When visiting a physician or other provider, always carry your insurance card and
present it to the receptionist.
- Be prepared to pay your out-of-pocket costs at the time of service.
- Make every effort to schedule needed appointments during regular office hours.
- Whenever possible, urgent care centers or physicians with extended office hours
should be used for urgent and non-emergency after-hours care.

Please note, changes to information contained in this directory may occur because of
additions, address changes, or changes in practice arrangements. Not all providers at a
plan facility may be contracted as Plan Providers. Remember to confirm your provider’s
status before receiving services. This can be done when you make your appointment.
